Purpose of This Consent

This document explains the nature of the medical services available through The Reset Regimen, the risks and benefits of these services, and your rights as a patient. By proceeding with clinical services, you acknowledge that you have read and understood this information.

Nature of Services

The Reset Regimen provides a physician-reviewed alcohol-support assessment that evaluates your current health status across multiple clinical dimensions including alcohol exposure, craving patterns, withdrawal risk, biomedical complexity, psychiatric safety, functional impairment, and environmental factors. Based on this assessment, you may be routed to appropriate care ranging from emergency referral to outpatient physician oversight to lower-acuity support.

Telehealth Disclosure

Services may be provided via telehealth (electronic communication). Telehealth carries certain risks including, but not limited to: the possibility of technical failures, limitations in the ability to perform a physical examination, and potential breaches of confidentiality. You have the right to refuse telehealth services and request an in-person consultation.

Risks and Benefits

Benefits: Access to physician-reviewed assessment, personalized care routing, ongoing monitoring, and evidence-based protocol recommendations.

Risks: The assessment may not capture all relevant medical history. Care recommendations are based on information you provide and may not account for undisclosed conditions. Alcohol withdrawal can be life-threatening and may require in-person emergency care that the Platform cannot provide.

Your Rights

  • You may withdraw consent at any time without affecting your right to future care.
  • You have the right to ask questions about any aspect of your care.
  • You have the right to a second opinion from another provider.
  • You have the right to know the qualifications of your reviewing clinician.
  • You have the right to refuse any recommended treatment.

Emergency Situations

If at any point during or after the assessment you experience severe withdrawal symptoms, suicidal thoughts, or any other medical emergency, call 911 immediately. The Platform includes safety detection that will interrupt the assessment and direct you to emergency services when appropriate.
